De continue kans op luchtbellen bij het aanbrengen van het materiaal is echter een bron van ergernis voor de behandelaar": zegt Dr. David Clarke. ###### Dr. David Clark heeft met 25 jaar klinische ervaring in zijn eigen praktijk, door laboratoriumonderzoek en honderden lezingen en hands-on cursussen vernieuwende matrixsystemen en -methoden weten te ontwikkelen die de kijk op restauratieve tandheelkunde veranderen. ### De huidige rol van flowable composieten Toen 3M in 1964 de eerste hars gemodificeerde, tandheelkundige composieten introduceerde, werden deze vooral gezien als vervanging van amalgaam. Omdat we dit gewend waren, dachten we dat we composiet op dezelfde manier laag voor laag moesten aanbrengen, modelleren en verwerken.  Toen in 1996 de eerste flowable composieten werden geïntroduceerd,1 wist vrijwel niemand wat men er mee aan moest, hoewel de voordelen van een vloeibaar materiaal dat onmiddellijk het tandweefsel bevochtigt duidelijk waren. Sindsdien hebben we geleerd het manipuleren van composieten, tegelijk betekent dat we meer uitdagingen kunnen tegenkomen.  Tegenwoordig wordt composiet vrijwel moeiteloos, zonder veel druk aangebracht. Een flowable is hierbij een ideaal, injecteerbaar materiaal.  ### Het probleem met luchtbellen De belangrijkste klachten die Dr. Clarke van behandelaars hoort over flowable composieten, gaan over luchtbellen die tijdens het aanbrengen ontstaan. Deze luchtbellen zijn soms zeer moeilijk weer te verwijderen of door te prikken, vanwege de viscositeit van het materiaal. Op beide elementen zijn luchtbellen te zien. ### Twee situaties waarbij luchtbellen te laat worden ontdekt. 1\. Zichtbare luchtbellen op röntgenfoto’s Bij de follow-up van een endobehandeling zijn twee luchtbellen zichtbaar op het element. Dat is pijnlijk en moeilijk aan patiënten uit te leggen. Ze zijn een integraal onderdeel geworden in de moderne praktijk van de meeste restauratief tandartsen. Nu flowable materialen en technieken steeds verder verbeteren, zie ik ook veelbelovende nieuwe doseringssystemen. Ik heb de nieuwe 3M spuit voor flowable composieten geëvalueerd.
